- For a note on the contents of this book, see BM 15th cent. where it is treated as a Low-Saxonn version of the Herbarius. Madsen lists as Promptuarium medicinae. Title page has a woodcut illustration of doctors attending a patient. Chancery folio, with paper measuring 28.4 x 20.5 cm. Text in two columns. Spaces left for initials. Signatures: [*⁸ a-b¹⁰ c-n⁸ o-p⁶]. Imperfect: leaf [*]1 (the title page) lacking. Initials in red or blue; rubrication in red. Binding: contemporary German blindstamped calf over wooden boards; remnants of two clasps.

Extensive contemporary ms. annotations on inside of front cover, on free endpapers at beginning and end, and on inside of back cover. Occasional annotations in the margins of the text.
