    Two completed printed forms, filled in, dated 1865; one form is signed E. O. Welch and dated August 29, 1864, the other form is completed but not signed or dated. The forms carry the heading: State of New York. Bureau of Military Record. Col. L. L. Doty, Chief of Bureau. Office of War Prisoners' Division, 37 Park Row, New York, 1865. The forms include questions about the soldier's experiences as prisoners of war, food, treatment, medical care, state of health, comrades, deaths, and any cases of shootings by Rebel Guards. The printed questions are followed by autograph text with additional information about their experiences during the war and as prisoners.
